Lip Enhancement By Filler

Overview of the surgery:
Lip enhancement filler can usually plump thin lips giving a voluptuous look. The most common way to increase lip size is with injectable fillers. The most common types are fat, collagen and restylane.

For collagen injections, a skin test is performed weeks before the procedure. This helps determine if any allergic reaction could possibly occur. If the test finds there is no risk of allergy, the collagen is then injected. During fat injections, both the donor and the recipient sites are treated with anesthesia. Then, fat is withdrawn with a cannula (a surgical suction instrument) or a syringe. When harvested and prepared, the fat is injected into the patient’s lips.

For Restylane, no skin test is required. The gel is injected into the inside of the upper or lower lips in tiny amounts with a very fine needle. As a result, it adds volume and shape to the lips

Duration of the surgery:
Lip augmentation with injectable fillers is a quick and easy procedure.  It takes about 15-30minutes.

Postoperative care:
The lip augmentation recovery process following lip enhancement with injectable lip fillers generally requires a day’s rest before patients can return to normal activities. After any type of lip enhancement procedure swelling and bruising can be expected.  Post-operative discomfort can be controlled with over-the-counter pain relievers.

Recovery period:
The results of lip augmentation using injectable fillers are apparent immediately. Your initial appearance after treatment with any filler may include:
·         Mild swelling or bruising
·         Temporary numbness or redness
·         An “over-filled” appearance to treated areas
These conditions can be alleviated with topical icing and will improve within a matter of hours or just a few days. When fat is the injected filler, these conditions may persist for a few weeks.

Possible risk and complications:
Complications from fillers are uncommon. The risks vary depending on the specific filler used and the relative permanence of the filler substance. Complications includes: Infection at the injection site, asymmetry, bleeding, bruising, and swelling, skin might die (skin necrosis), skin rash, itching, skin redness, skin sensitivity.

Anesthesia use:
Anesthesia is typically not necessary for the procedure because the filler usually includes lidocaine or another topical anesthetic.

Hospital admission:
It depends on your preferences as well as your surgeon’s. Lip enhancement filler can be performed on an out patient basis, in the hospital, or in an ambulatory surgical suite.

Preparation before Surgery:
You and your surgeon will discuss any past treatment with injectable fillers, facial shaping or Botox therapy or other facial rejuvenation procedures, medical conditions, drug allergies and medical treatments. The doctor will also evaluate your general health status and discuss the options available to you for facial rejuvenation.
  • Assessment of the medical history (any allergies, serious medical condition and all medications taken both prescribed and non-prescribed), physical examination, and laboratory tests will be performed during consultation.
  • Blood and urine samples will be collected for routine preoperative laboratory tests.
  • Smoking must be avoided for about 3-4 weeks prior to surgery, as nicotine interferes with circulation and will greatly affect healing process.
  • You will likely to be asked to stop drinking alcohol, a week before the surgery and throughout your recovery period.
  • Avoid taking any medications such as hormones, anticoagulants, anabolic steroids and supplements at least 4-6 weeks to prevent complicating medical factors prior to surgery. Avoid taking aspirin, anti-inflammatory drugs and herbal supplements as they can increase bleeding.

Lip Enhancement Surgery ( Lip Augmentation)

Overview of the surgery:
Lip Enhancement Surgery (Lip Augmentation) A thin lip can be augmented with the patient’s own tissue (fat, anadesma), with implantation of an artificial material (collagen, hyaluron acid) or by a surgery using the tissue in vicinity of the lip.
Lip implant surgery, begins with creating a tunnel to accommodate the implant. The implant is then clamped with an insertion device that usually comes with it, your lip is skewered and the implant is pulled through the lip. The implant(s) are trimmed to size and the incisions stitched closed.

Duration of the Operation:
15 minutes to 2 hours. Most procedures are finished in 1 hour or less.

Post operative care:
Return to normal activities: usually within a day. Keep ice packs applied to your lips for the first 48 hours and limit talking and chewing. To avoid unnecessary swelling or bleeding, you should not bend over, strain, exercise or do any activities that could increase pressure to your lips. Keep the lips clean to avoid infection.

Recovery period:
Your recovery will depend on your lifestyle and which procedure you undergo. If you are having injections, your recovery can take just a couple days. However, any swelling or bruising can last longer. If you are having surgical implants or grafts, your recovery can take up to two weeks after the surgery. You can go back to normal activities usually within a day but graft recipients require 1-2 weeks.

Possible risk and complications:
Common reaction can range from redness, swelling or itching at the injection site(s). Other possible complications include bleeding, uneven lips, movement of the implants or extrusion, infection or nerve damage. The usual, expected swelling and bruising can last from several days to a week. As with any surgery, there are risks and complications. You can have allergic reactions to implants as we've described above or allergic reaction to anesthesia. It’s very important that you discuss any concerns with your doctor prior to surgery and that you make your doctor aware of any existing medical conditions which may cause further complications.
Anesthesia use:
The procedure usually uses Local anesthesia

Hospital admission:
It depends on your preferences as well as your surgeon’s. Lip augmentation can be performed on an out patient basis, in the hospital, or in an ambulatory surgical suite.

Preparation before Surgery
  • Assessment of the medical history (any allergies, serious medical condition and all medications taken both prescribed and non-prescribed), physical examination, and laboratory tests will be performed during consultation.
  • The surgeon will also check your mouth. If you suffer from any of the following active lip or oral cavity infections, you will have to postpone the procedure (Herpes, cold, tonsillitis).

Lip Reduction

Overview of the surgery:
Lip Reduction, also called reduction cheiloplasty, is the procedure to remove excess lip tissue to reduce the appearance of overly large lips.
Lip reduction is performed by making an incision in the length of your lip inside your mouth and then removing a strip of skin from one or both lips, depending upon the look you want to achieve. Your Surgeon will aim to enhance the mouth and lips area to complement your facial features, giving you a softened, natural mouth.

Duration of the operation:
The procedure takes only about 30 minutes - 1 hour.

Post operative care:
You may notice a change in your smile, odd sensations of “hollowness”, tingling, the sporadic sharp pain, or “pulling”, burning, and cold sensation. Take it easy and sleep on two pillows to keep your head elevated for around 7-14 days or how long your surgeon suggests. Remember that swelling and bruising id perfectly normal after surgery but this will clear up after a few weeks. You should expect to see the final results from lip reduction surgery in about 3 months. Because of the swelling your lips may look larger after the procedure than before but they will settle in time

Recovery period:
Your swelling will subside within a few weeks, but usually the final result will be visible after about 3 months of the surgical procedure. These usually subside within the first few weeks. You can return to work at 5 to 7 days – but it depends upon type of your work. If you have a sedentary (desk job) with little or no amount of talking it is ok to resume your work after about a week.
Give yourself a week after the completion of your lip reduction surgery returning to your normal routine. It’s also advisable to refrain from participating in contact sports for at least six to eight weeks.
Possible risk and complications:
All surgery has risks and complications. With lip reduction, these risks include allergic reaction to the anesthesia and infection. There is also the risk of asymmetry and/or cosmetic dissatisfaction. Numbness is also possible, but this usually subsides within the first few weeks. Excess scar tissue and lumps in your newly-thinned lips may also occur. If your lips are too thin as a result of lip reduction surgery, lip augmentation may help correct this.

Anesthesia use:
The procedure can be performed under local and or regional anesthesia although some surgeons may choose to perform it under Light Sleep anesthesia.

Hospital admission:
It depends on your preferences as well as your surgeon’s. Lip reduction can be performed on an out patient basis, in the hospital, or in an ambulatory surgical suite.

Preparation before Surgery
  • Assessment of the medical history (any allergies, serious medical condition and all medications taken both prescribed and non-prescribed), physical examination, and laboratory tests will be performed during consultation.

Old Fractured Nose Surgery

Overview of the surgery:
Old Fractured Nose is a break or crack in a bone in your nose, often the bone over the bridge of your nose from severe accident which can cause serious breathing problems and crooked appearance that you might probably consider rhinoplasty. This is the procedure that surgeons most often make use of in order to fix patient’s nose and restore it to its former appearance. If you are unhappy of the results after the surgery, you can go for a revision rhinoplasty, where the surgeon can rectify the work done on your nose previously to have it restored to how it was originally was.
An incision through the outer nose is rarely required, since access can be gained through the nostrils. However, the septum will be incised to begin the reshaping process. The septum is straightened through partial removal or readjustment of the affected bone and cartilage. Cartilage will be incised, shaved, or sliced to make the septum lie in the middle of the nasal vault. Sometimes the cartilage in a malformed septum is altered until it is sufficiently weakened, and then the septum is built up again with autologous cartilage from another site in the individual's body. Bone is removed rather than shaped. The goal is always to correct the deformity while supporting the nose structure. In some cases, septoplasty will be done at the same time as reshaping of the outer nose (rhinoplasty).

Duration of the surgery:
The procedure usually takes around 1 hour or depending on the extent of correction.

Post operative care:
You need to get adequate rest in order to recover. A plastic splint molded to your nose will help maintain alignment.  Your air passage maybe temporarily blocked with gauze to control the bleeding.   The frames of glasses must be avoided to rest on the bridge of the nose for three weeks.

Recovery Period:
You should avoid blowing your nose altogether for the first 10 days. If there is swelling, cold showers can help reduce it to a certain degree. During this time, you will refrain from any strenuous activities as well as sneezing.

Possible risk and complications:
Infection is rare, but if it occurs it can cause significant problems. Lacerations cause scars. External scars sometimes need surgical revision. Internal nasal scars and deformities can increase your chance of nose bleeds. Damage to nerves can result in altered, diminished, or painful sensation or poor muscle function (paralysis). Injuries may obstruct the small holes that drain your sinuses. Pressure in sinuses can result in painful headaches.

Anesthesia use:
The procedure is performed with local, IV sedation or general anesthesia.

Hospital admission:
It depends on your preferences as well as your surgeon’s. Old fractured nose can be performed on an out patient basis, in the hospital, or in an ambulatory surgical suite.

Preparation before Surgery:
  • Assessment of the medical history (any allergies, serious medical condition and all medications taken both prescribed and non-prescribed), physical examination, and laboratory tests will be performed during consultation.
  • Blood and urine samples will be collected for routine preoperative laboratory tests. X-rays may be taken to evaluate the individual's bone structure.

Deviated Nose ( Septosplasty)

Overview of the surgery:
Deviated Nose (Septoplasty) is an operation to correct a deformity of the nasal septum. The usual purpose is to improve nasal breathing, but it may also be performed to allow adequate examination of the inside of the nose for treatment of polyps, inflammation, tumors, or bleeding. When the nasal septum is deformed, there is no medicine that will cause it to be straightened, so surgery is the only solution to this problem.
A small incision is made inside the nose. The mucosal lining of the septum is detached from the cartilage and bones of the septum. The deviated portions of the septum are removed or straightened. The nasal lining mucous membrane is replaced.

Duration of the operation:
Septoplasty surgery to straighten the septum or remove the obstructing anatomy takes about an hour of operation.

Post operative Care:
Splints or packs are placed in the nose for a few days to help the septum to heal into its new position. These splints / packs have a straw that allows the patient to breathe from the nose and makes swallowing much easier. There are no external bruises or swelling of the eyes because the outer nasal bones are not broken or interfered with as in cosmetic nasal surgery. If an antibiotic is prescribed then it should be strictly followed to prevent infection.  Saline irrigation of the nasal cavity helps in speeding up healing process.

Recovery period:
The septoplasty recovery time normally ranges from one week to three weeks. Patient is advised to blow his nose only after a day or two and that too very gently. Only after a week or two are patients asked to blow their nose normally.

Possible risk and complications:
The undesirable results that may occur include: a hole in the septum, failure to completely improve breathing, bleeding (usually easy to control), wound infection, nasal crusting, and very rarely, a change in appearance. . There are risks with all surgical procedures. You should discuss all risks with your cosmetic surgeon.

Anesthesia used:
The procedure is performed with local, IV sedation or general anesthesia.

Hospital admission:
The patient can be discharge from the hospital on the same day of the surgery.

Preparation before Surgery:
  • Assessment of the medical history (any allergies, serious medical condition and all medications taken both prescribed and non-prescribed), physical examination, and laboratory tests will be performed during consultation.
  • Smoking must be avoided for about 3-4 weeks prior to surgery, as nicotine interferes with circulation and will greatly affect healing process.

Alar Resection

Overview of the surgery:
Alar Resection is a cosmetic surgery procedure of the nose that narrows the wide flaring and large opening of the nasal nostrils.
The alar of the nose is reduced and suture in its position, aim is to make the balance of nasal tip and alar of the nose.

Duration of the operation:
The procedure usually takes around 1 hour.

Post-Operative Care
Splint the nasal tip with 3-4 mm thin strips of waterproof tape. Steri-Strips also can be used. Run the strips along each side of the nose and gently wrap them under and around the tip. As all sutures are absorbable, none require removal. Revision surgery should not be considered before at least 6 months have elapsed since the original operation.
Recovery period:
Normally, you will feel some pain and discomfort at the nose area during the first days after the operation but it can be alleviated by the medications prescribed. After 2 or 3 days, you will feel no pain but swelling in the nose and eyelid area will persist. Swelling and bruising around the eyes will start to disappear within a week and will completely subside for about 2 weeks. You can go back to work after 1 or 2 weeks, but avoid strenuous activities that can increase pressure to the face, which can prolong swelling and bruising for the next 2 to 3 weeks. After the removal of the bandage and stitches, the patient must take care of her/his nose not to be injured until it is completely healed. The frames of glasses should not be allowed to rest on the bridge of the nose for three weeks. A little bleeding is common during the first few days following surgery; patient may continue to feel some stuffiness for several weeks. Patient must try not to blow nose for a week or so, while the tissues heal.

Possible risks and complications:
Serious complications are rare. The most common complication is patient dissatisfaction. Other complications include: hematoma, airway obstruction, injury to the nose, infection, excessive swelling and bleeding.

Anesthesia:
Using IV sedation (medicine administered through an IV) and local anesthesia (numbing medication administered in the surgical area)

Tip Plasty and Tip Graft

Overview of the surgery:

Tip Plasty and Tip Graft may be performed to reduce a large, square, or bulbous nasal tip, to treat an upturned or sagging nasal tip, or just to change the appearance of the nasal tip.
The surgery includes realignment of the alar cartilage by resection and suture, fibroareolar and subcutaneous tissue, tip graft and columellar strut. The nasal tip is positioned high and elegant, methods such as cartilage suture, cartilage grafting and blocking the nasal septum.

Duration of the procedure:
The procedure usually takes around 1 hour depending on the surgical treatment and the extent of reshaping required.

Post operative care:
You will feel some pain and discomfort at the nose area during the first days after surgery but can be alleviated by medications prescribed. Expect some bruising and swelling during first few days and will usually subside in about 1 week.

Recovery period:
The typical recovery would be around 3 weeks. Sutures are removed within 10 days after the procedure.

Possible risks and complications:
These are complication free. However, all grafts have potential for asymmetry and migration. Although this may seem like a simple operation because it involves such a small area, a tip plasty may be one of the most challenging aspects of nose surgery. Disruption of the cartilage that supports the tip of the nose can cause the tip to collapse, rotate or lose projection; therefore tip plasty should be performed by an experienced rhinoplasty surgeon.

Anesthesia use:
This procedure is generally under local or general anesthesia.

Reduction Rhinoplasty (Nose Reduction)

Overview of the surgery:
Reduction Rhinoplasty is for patients who have prominent large noses with oversized bridges and depressed nasal tips. It aims to create a well balanced nose by reducing the dorsal hump, narrowing the bridge, improving the nasal tip projection and definition.
The incisions are placed on the rim of the alar of nose. Soft tissues of the nose are then separated from the underlying structures, the bone and cartilage are then reshaped and the skin re-draped over the surface. Patients who have a deviated septum may benefit by correction of this problem at the time of surgery.

Duration of the operation:
The procedure usually takes about 1-2 hours.

Post operative care:
The nose will be covered with either plaster or a light metal or plastic splint to maintain the position of the nasal bones.  You will feel some pain and discomfort at the nose area during the first days after surgery but can be alleviated by medications prescribed.

Recovery period:
You can go back to work or return to school within 2 days following surgery and will take about 1-2 weeks until it will be completely healed.

Possible risks and complications:
When Nose Reduction is performed by a qualified plastic surgeon, complications are infrequent and usually minor. Nevertheless, there is always a possibility of complications, including infection, nosebleed, or a reaction to the anesthesia. You can reduce your risks by closely following your surgeon's instructions both before and after surgery. After surgery, small burst blood vessels may appear as tiny red spots on the skin's surface; these are usually minor but may be permanent. As for scarring, when Nose Reduction is performed from inside the nose, there is no visible scarring at all; when an "open" technique is used, or when the procedure calls for the narrowing of flared nostrils, the small scars on the base of the nose are usually not visible

Anesthesia:
Local anesthesia with mild sedation

Hospital admission:
It depends on your preferences as well as your surgeon’s. Reduction Rhinoplasty can be performed on an out patient basis, in the hospital, or in an ambulatory surgical suite.

Preparation before Surgery
  • Assessment of the medical history (any allergies, serious medical condition and all medications taken both prescribed and non-prescribed), physical examination, and laboratory tests will be performed during consultation.
  • Smoking must be avoided for about 3-4 weeks prior to surgery, as nicotine interferes with circulation and will greatly affect healing process.

Augmented Rhinoplasty (Nose Augmentation)

Overview of the surgery:
Augmented Rhinoplasty is very common among Asians and non-Caucasians to correct a flat nose with a round bulbous tip and flaring nostrils. The aim is to build an aesthetically pleasing nasal bridge with an elegant projection and well defined nasal tip, and at the same time narrow the flaring nostril base. An implant is usually used. The implanted material is biocompatible and extremely safe.
Incisions are made inside the rim of the nostrils. Soft tissues of the nose are then separated from the underlying structure to create the appropriate space for nose implant projecting the nasal tip and increase the height of the nose.

Duration of the procedure:
Rhinoplasty generally takes about 1 to 2 hours to complete.
Post operative care:
The nose will be covered with either plaster or a light metal or plastic splint to maintain the position of the nasal bones. You will feel some pain and discomfort at the nose area during the first days after surgery but can be alleviated by medications prescribed.

Recovery period:
Most nose augmentation patients are able to return to school or work after surgery within 2 days. However it will be about 2 weeks you’re entirely healed.

Possible risks and complications:
Rhinoplasty will provide significant improvements in the appearance of the nose for the vast majority of patients although, in occasional cases, a secondary correction may be required. There are risks with all surgical procedures, including infection, nose bleeding, or a reaction to the anesthesia.

Anesthesia use:
Using IV sedation (medicine administered through an IV) and local anesthesia (numbing medication administered in the surgical area)

Hospital admission:
It depends on your preferences as well as your surgeon’s. Augmented Rhinoplasty can be performed on an out patient basis, in the hospital, or in an ambulatory surgical suite.
